We offer patient assistance for many of our services to help make healthcare more accessible. However, please note that our pain management services do not have assistance available, and copays and coinsurance will be required at the time of service.

If we are treating you for the condition related to your disability request, we can schedule an appointment to discuss completing the paperwork. Please note that there is a processing fee of $35.00 for filling out the paperwork.

By law, our medical records department has up to 30 days to fulfill your request. However, we strive to provide your records as quickly as possible. If you have a specific deadline, it’s helpful to note that date on the release of information form.

No, by law, we can only release our own records to you. Any records sent to us from another provider or facility must be obtained directly from that office.
